General Instructions - Weekly Activity Log: 

Your Weekly Activity Log will have to be electronically filled out and submitted weekly. Use your copy of the practice log from Module 1 as a reference and a guide. I suggest that you keep a paper log at home that looks just like the practice one. The Weekly Activity Log cannot be opened repeatedly, it must be filled out and submitted all at once at the END of the WEEK (Friday, Saturday or Sunday). You must exercise 5 days a week for no less than 10 minutes a day. At the end of 5 days you must have exercised a total of 150 minutes. Each week you will have one calendar week to accumulate the 5 days of physical activity.

Assignment Instructions:

  1. Refer to your copy of the practice log from Module 1 to guide you  
  2. Choose an activity or group of activities to participate in for 5 days
  3. Activities are supervised by an adult, preferably one with extensive knowledge about the activity and is CPR certified, or your PE teacher
  4. Each activity day you will need to: a) log the name(s) of the activity and b) amount of time in minutes you were active
  5. You will also need to self assess your pre-exercise heart rate (once a week) by manually taking a pulse rate or by a monitor; on the same day you will also need to self assess your post exercise (0-2 minutes after exercise has been ended) heart rate (also once a week) in the same method used for the pre-exercise heart rate
  6. Once you have exercised for a total 5 days you are ready to login to Canvas and submit your WEEKLY ACTIVITY LOG 
  7. In Canvas, go to the course screen and select the correct activity log for the current MODULE and open it
  8. Read the instructions and answer every question completely
  9. Then, submit your Weekly Activity Log by left clicking the "Submit Quiz" button at the bottom of the page

Grading: 15 Points - Timed 30 minutes to submit your Weekly Activity Log

  1. Activity Data = 10 points (each reported activity day is scored as: 1 point name of activity, 1 point time active)
  2. Intensity Scale = 1 point
  3. Pre-Exercise heart rate = 1 point
  4. Post Exercise heart rate = 1 point
  5. Short-term Exercise Goal Statement = 2 point
